We are ready to register Kindergarten students for August 2010!!   

  Please see the information below regarding what you need to bring and stop by the school to register.  Due to limited office personel, we advise that you call first before coming  (248-2302). 

KINDERGARTEN REGISTRATION -  Please bring with you the following required items:

1. Florida Shot Record (form 680) 
2. Proof of current address (TWO of the following)
     Rent or lease agreement;   contract on home being purchased; utility receipt or bill with your name on it   (electric, water, gas or phone)
   -- If you live at a residence other than your own, you must provide all of the following: 
   A notarized statement from the person you reside with verifying that you and the student live at that address
    A notarized statement from yourself stating that you and your child live at that address.
   One of the other items listed above under #2 “ Proof of current address”
3. Social Security Card or letter from the Social Security Office that reflects the Social Security Number  (by request only - not required)
4. Birth Certificate
5. Physical (must be within a year prior to the date school starts)
   
   Once you have provided the information above, Mrs. Conner will call you to give you your child's student ID number.  After receiving that information, you will need to register your child on-line for Open Enrollment to complete your registration at www.bay.k12.fl.us/schoolchoice    If you do not have access to a computer, a designated computer will be available at Deer Point Elementary, the Nelson Building or at the Lynn Haven Public Library.
